Purpose:

The Licensed Practical Nurse will facilitate the well-being of youth while providing nursing care and health education. Services will be provided with the highest ethical standards of the nursing profession, complying with LUK’s mission, vision, and values as well as regulatory/licensing/funder requirements.

Responsibilities Include:

  • Provide proper medical care to clients within the scope of LPN practice and in compliance with applicable state, federal, and other regulatory laws
  • Follow up on nursing care plans as written by the Residential Services Division RN
  • Complete nursing assessments for new client intakes, as directed by the Division RN
  • Oversee non-MAP certified programs’ medication logs
  • Assist in obtaining proper medication orders for clients
  • Assist in scheduling all necessary medical and dental appointments
  • Take identified clients to their appointments
  • Maintain ongoing communication with medical providers to ensure comprehensive service provision
  • Assist with staff training in medication management and in the proper care of clients
  • Coordinate with the RN to coordinate/provide health education for clients to maintain well-being and an optimum level of health care while building transitional living skills
  • Document nursing activities according to established guidelines
  • Attend Division and client-specific meetings as needed
  • Participate in the Continuous Quality Improvement Process
  • Maintain nursing licensure requirements
  • Attend and participate in scheduled supervisions and LUK trainings
  • Participate in the Placement On Call System as scheduled
  • Gain an understanding and commitment to LUK’s Mission, Vision, and Values and provide congruent services

Knowledge and Experience Required:

  • Licensed Practical Nurse in the Commonwealth of Massachusetts
  • 3-5 years of direct care experience
  • Bilingual/bicultural candidates encouraged to apply

  • Driver’s license along with reliable and safe transportation
