Purpose:

The CFSS Case Manager provides strength-based services to young people and their families to support progress toward their goals.

Responsibilities Include:

  • Meet with young people and their families at their home, school, or other community-based location regularly
  • Maintain communication with young people, their families, and other treatment team members between meetings
  • Coordinate with the program coordinator to complete intakes and discharges in a timely manner, to include screening and assessments as well as individual treatment/action plan development
  • Identify needs of the participants and their families and connect them to community resources to meet those needs (warm hand-off preferred)
  • Complete regular reviews of progress toward goals to quantify participant functioning
  • Model skills for young people and families and coach/mentor them to develop their own skills
  • Actively participate in supervision and training as required
  • Provide services that respect each family and youth’s cultural background, heritage, and identity
  • Participate in emergency services rotation
  • Gain an understanding and commitment to LUK’s Mission, Vision, and Values and provide congruent services

Knowledge and Experience Required:

  • Bachelor’s Degree in Human Services, Counseling, Social Work, Psychology or closely related field
  • 1 years of experience providing case management
  • Bilingual/bicultural candidates encouraged to apply

  • Driver’s license along with reliable and safe transportation
