Job Description
The Post Graduate Employment & Community Specialist supports young adults with disabilities in developing employment, independent living, and community participation skills. This position provides individualized job coaching, workplace support, and transition services to post-graduate students and high school students with life skills needs as they prepare for successful adult outcomes.

The work year is the school year (180 days) plus four weeks during the extended school year (summer), for a total of 198 days.


 

  • Field work with students and local businesses
  • Consistently model appropriate work maturity skills and hold graduates to the same expectations.
  • Provide services to graduates without discrimination as to race, religion, sex, age, sexual orientation, or any other disabling condition.
  • Monitor the graduate’s work performance and provide feedback and prompts when needed.
  • Provide proper documentation on all graduates’ work performance.
  • Ensure safety is being maintained in the work environment at all times.
  • Know and adhere to established agency policies, procedures, and childcare regulations.
  • Uphold and consistently follow program and licensing regulations and procedures carefully for the graduate’s place of employment.
  • Maintain and model positive, professional working relationships with consumers and other agency staff.
  • Accept and integrate supervision. Monitoring by case notes and monitoring checks.
  • Through work performance evaluations, demonstrate a solid working knowledge of the most frequently encountered disabilities of graduates and the ability to provide specific, quality service for them.
  • Willing and able to work with students with incontinence and/or personal care, in a respectful and professional manner when necessary.
  • Provide service for graduates regardless of location by creating and maintaining an atmosphere that is welcoming, understanding, communicative, and supportive of consumers.
  • Attend required team meetings and professional development training sessions.
  • Write transition goals and benchmarks for IEPs and quarterly progress notes.
  • Teach job skill training in the high school life skills classroom in conjunction with the special education teaching staff.
 
Position Type: Full-time
Salary: $30 Per Hour

Job Requirements
  • Associate's Degree preferred
  • Possess and maintain a current CPR/First Aid Certificate
  • Minimum one-year experience delivering supported employment services or direct care services either in a classroom, place of employment, day care, medical, or residential settings.
  • Associate degree preferred.
  • Citizenship, residency or work visa required
