* Work schedule may vary based on patient needs. Work schedule may require Saturday availability with days off being on Sunday and another day during the week. Role will work at least 50% of time at Leominster campus and then the remainder of time at University campus*

 

I. Major Responsibilities: 

1. Oversees and assists other health coaches and BHS volunteers on procedural tasks and project expectations.
2. Conducts screening for a variety of behavioral health behaviors, conducts brief interventions, motivational interviewing, and refers patients to appropriate resources for education and or treatment within the Emergency Department. 
3. Documents all information obtained in patient evaluation, assessment, consultation, referral and disposition to comply with regulatory requirements, as well as ensuring best possible patient outcome.
4. Collaborates with inpatient clinical team and interdepartmentally with psychiatry to ensure successful transition of care through the entire healthcare system after admission from Emergency Department to inpatient status.  
5. Participates in the evaluation of health and education programs including wellness, nutrition, physical fitness, tobacco and unintentional injuries.
6. Engages patients as appropriate in support activities relating to behavioral and necessary primary care.
7. Works collaboratively with providers including, but not limited to, physicians, nurse practitioners, nurses, and other health care professionals both within UMMH and community resources.
8. Recommends and assists in the implementation of goals and objectives; develops logical scope and sequence plans for providing health education programs.
9. Attends trainings on current, relevant clinical practices and subsequently trains other health coaches on important practices and potential changes to workflow processes.   
10. Acts as a resource person in health education; applies appropriate research principles in health education.
11. Attends weekly interdepartmental meetings.
12. Aids in presenting findings from projects and initiatives to Program Manager, Program Director, specific committees, at local and national conferences, and others. 
13. Assists in the evaluation of utilization and impact of the BHS.
14. Assists in identifying additional resources for referrals and patient educational resources when necessary.

15. Leads or facilitates trainings, workshops, and educational series related to substance treatment and best practices.
16. Adheres to UMass Memorial compliance policy, departmental guidelines/policies and all applicable laws and regulations.

 

 

II. Position Qualifications:

 

License/Certification/Education:
Required:
1. Bachelor’s degree in psychology, social work, or medicine

2. Licensed mental health, substance abuse or additional counselor

 

Preferred:
1. Lean Yellow Belt training within one year of hire or promotion into the position

 

Experience/Skills:
Required:
1. 2+ years of psychology, social work, or human services experience.
2. Knowledge and experience using the current DSM and/or ICD codes.

3. Knowledge and support of current evidence-based treatments for substance use disorders and appropriate levels of care with respect for different approaches to recovery.
4. Data compilation and analysis skills. 
5. Demonstrated ability to establish and maintain rapport with patients, hospital staff and co-workers from diverse backgrounds.
6. Ability to work with people in a demanding environment with high patient flow.    
7. Knowledge of basic medical terminology.  
8. Knowledge of HIPAA regulations.
9. Must be able to work independently, communicate medical information and perform health interventions in a clinical setting.  
10. Experience using electronic health record, Microsoft Office products, and computers. 
11. Excellent written and communication skills.

 

Unless certification, licensure or registration is required, an equivalent combination of education and experience which provides proficiency in the areas of responsibility listed in this description may be substituted for the above requirements.
