We are hiring Homecare Nurses - full time positions available. We care for mental health patients of all ages and backgrounds who require daily visits in their home:

RN Full Time AM Shift in Fitchburg/Leominster and the surrounding areas, up to $80K annually plus incentive pay.

What we're looking for in a Nurse

To thrive as a Nurse with Comfort Home Care, you'll need a diverse skill set and a proactive mindset. Your expertise in conducting initial and ongoing assessments is crucial for determining each patient's medical needs and communication abilities. Being adept at initiating and revising care plans will help you provide tailored interventions that significantly enhance patient outcomes.

Collaboration is key, too! You'll engage with families, coordinate services, and keep physicians informed about any changes in your clients' conditions. Additionally, your ability to perform OASIS assessments at the right time is essential for tracking progress.

You'll also need to prepare detailed clinical notes and manage required paperwork with precision and timeliness. If you're a smart, empathetic problem-solver who embraces innovation and values integrity, this role is perfect for you!

Knowledge and skills required for the position are:

  • RN provides initial and ongoing assessments to identify the client's medical needs level of functioning and ability to communicate and learn.
  • RN initiates the plan of care necessary revisions and performs OASIS assessments at appropriate time points when applicable.
  • RN initiates and performs appropriate nursing interventions.
  • RN coordinates services and informs the physician and other personnel of changes in the client's condition and needs.
  • RN collaborates with patient /family and other health care providers and/or community resources with planning of care and discharge.
  • RN prepares clinical and progress notes and submits all required paperwork in a timely manner according to agency policy and procedure.
