Responsibilities:
Provides comprehensive social work, case management and advocacy services for the individuals and families on their caseload, and will serve as therapist for assigned adult individuals. Assists in the development of short-term and long-term goals, contributes to and attends Individual Service Planning conferences working in conjunction with the program nurse. Provides on-going and consistent contact with families as to progress and treatment needs, acts as MassHealth contact for individuals. Maintains interdisciplinary contact with psychiatry, nursing, residential programs and vocational or day placement.
Requirements:
Requires Master's Degree in Counseling or Social Work, independent licensure preferred, and a minimum of one-year clinical experience. Requires working knowledge of Food Stamp Program, eHana and HCSIS systems.
Monday-Friday, 8:00 a.m. to 4:00 p.m., with some flexibility.
