Description

  

POSITION SUMMARY:

Responsible for documentation and maintenance of assigned participant assessments, records, plan of care, and suitability of living arrangement. Must proactively focus on consumer advocacy, empowerment, and community integration. 

RESPONSIBILITIES:

  • Conducting the initial and ongoing assessments of the AFC and/or SL provider setting      and maintain appropriate documentation.
  • Obtaining a social history and conducting a psychosocial assessment and evaluation.
  • Select, train, evaluate and supervise AFC caregivers in conjunction with the registered      nurse.
  • Participate in the development, implementation, and ongoing review of the AFC plan of      care.
  • Review and document the fire and safety procedures of the provider setting annually      or more often as indicated, review the continued suitability of the      provider setting.

· Conducting assessments and maintaining appropriate documentation including, but not limited to:

· Conducting the initial and ongoing assessments of the AFC and/or SL provider setting.

· Obtaining a social history and conducting a psychosocial assessment and evaluation of the provider and his/her family (if applicable)

· Review and document the fire and safety procedures of the provider setting. 

· Annually, or more often as indicated, review the continued suitability of the provider setting.

  • Conducting      on-site visits bi-monthly with each Level I participant and monthly with      each Level II participant ensuring that each Level I participant is seen      each month.
  • Completing a      care management progress note corresponding with each on-site visit      encounter and upon any significant changes.
  • Participate      in scheduled staff meetings, supervision and clinical meetings as      specified by supervisor.

· Supporting Choice and Decision Making, including but not limited to

Support individuals in self-advocacy and making meaningful choices about daily activities, meals, and leisure time.

  • Ensuring that      individuals are treated with dignity and respect in accordance with Minute      Man’s Human Rights Policy and DDS (Department of Developmental Disability)      regulations.
  • Assist      individuals in maintaining their privacy and confidentiality.
  • Support      individual’s ethnic and religious preferences.
  • Ensure      individuals have free access to all areas of the home, access to personal      possessions and the right to be free from harm or abuse.

As mandated reported, staff must understand DPPC guidelines and appropriately utilize the reporting process. Duties may also include filing out incident reports and writing notes in the communication log.

· Day to Day Duties include but are not limited to:

· Completing required reports and maintaining documentation/record keeping in accordance with agency and regulatory agency guidelines.

Report significant issues (health, behavioral, psychiatric etc.) to the appropriate supervisor in a timely fashion while also documenting issues and incidents in accordance with agency and cluster guidelines.

Perform physical interventions as required in a client crisis.

 

 

QUALIFICATIONS:

· Bachelor’s degree, in Social Work or at least two (2) years’ experience in working with elderly or disabled adults. Or:

Bachelor’s degree and two (2) years clinical experience in the care of elderly or disabled persons.

· Current certification in CPR, First Aid, helpful. Training provided.

· Valid driver’s license and reliable transportation.
