Homecare RN Case Manager – Full-time (32 or 40 hours- $3,000 SIGN ON BONUS) and Per-diem
Job Summary: As a member of the interdisciplinary team, the Homecare RN works under the direct supervision of the Clinical Supervisor. The Homecare RN will coordinate nursing assessment, planning and care in order to maximize the health of patients, provide professional in-home nursing care to clients, assess and evaluate client's status by reviewing and implementing the plan of care and regularly re-evaluating the client's needs, administer medications and treatments as prescribed by the physician. Observe client for changes in condition and discuss changes with the client, caregiver, and attending physician.
Job Requirements: Current RN MA Licensure, 2 years home health nursing experience preferred (not required), 2 years of medical/surgical, sub-acute preferred (not required), currently holds a valid driver’s license, ability to learn and be proficient with required documentation software (Home Care Home Base) using agency electronic device.
